What this website collects, and what it does not.
Short version: the assessment form, and nothing else. No trackers, no advertising cookies, no data sold.
Before launch: this notice describes how the site as built actually behaves, but it has not been reviewed by a lawyer. Have counsel check it against HIPAA, state privacy law and any analytics you add, and replace this box with a real effective date.

This notice covers https://www.drlorents.com only. Health information created once you become a patient of Dental Art Center Oklahoma City is handled under the practice's own patient privacy notice, which is given to you at your first appointment and is separate from this document.
What the website collects
The assessment form is the only place this site asks you for anything. It collects what you type into it and nothing else.
- Your name, so the assessment can be addressed to you
- Your email address, so the written assessment can be sent back
- Your phone number, only if you choose to give one
- The state you would be travelling from, and the treatment you are asking about
- Whatever you write in the message field
Do not send clinical records through this form
The assessment form sends an ordinary email. It is not a secure clinical channel and it should not be used for radiographs, scans, medical history or anything else you would not want sitting in an inbox.
Once an assessment is under way the practice will ask for clinical records through a secure channel. Wait for that request.
Why it is collected, and on what basis
Solely to answer your request. Submitting the form is a request for a service, and the details are used to provide it. Nothing submitted through this site is sold, rented, or passed to advertising networks.
How long it is kept
Assessment enquiries are kept for as long as the enquiry is active, and then archived under the practice record-retention schedule. If you ask for your enquiry to be deleted and you have not become a patient, it is deleted.
Who else can see it
The clinical and reception team at the practice, and the email provider that carries the message. No other third party receives it.
Cookies and analytics
This site sets no advertising cookies and runs no cross-site tracking. If analytics are added later, this notice will be updated before they are switched on.
Your choices
Write to care@drlorents.com or call (405) 769-3373 to ask what is held about you, to correct it, or to have it deleted. Depending on where you live you may have additional statutory rights, and the practice will honour them.
